Don’t forget to bring this form to your appointment along with all insurance cards, any glasses and/or contact lenses you have been using.

Flexible Spending Accounts
Does your employer offer a Flexible Spending Account (FSA) option? These funds can be used in our office to cover the cost of copayments, glasses, contact lenses, and medical visits. Please contact our office at 616-228-4790 with any further questions.
Vision Insurance
Signature Eye Care participates with most vision and medical insurance plans. If you do not see your insurance listed below, please call our office as these plans may have been updated. We are happy to verify your individual insurance plan when you call to schedule your appointment.
